Independent Occupational Therapist and Case Manager

Profile

I am an independent Occupational Therapist and Case Manager with more than 20 years’ clinical Occupational Therapy experience. I specialise in working with clients with physical disabilities, particularly those with hand and upper limb trauma, I also have extensive experience in the rehabilitation of individuals of all ages following complex orthopaedic injuries, including amputations and general medical conditions together with associated psychological effects.

I have had considerable experience working for the NHS for 19 years and more recently moved focus to working within the private insurance and personal injury claims sector. I carry a wide-ranging caseload with referrals from a variety of sources including insurance companies, solicitors, employers and medical consultants. I offer a comprehensive range of individually tailored services including assessment, rehabilitation and case management of clients with major and multiple trauma, fractures, amputations and chronic pain. I complete home and work place evaluations and provide advice on suitable adaptations and return to work packages. My clients range in age from teenagers through to older age adults

I liaise closely with all key stakeholders including medical professionals, insurers, solicitors, employers, disability employment advisors and social workers.

Clinical skills/experience

  • Extensive hands-on experience within the public and private sector. This has included assessment and clinical rehabilitation of clients of all ages with a wide range of physical conditions including multiple and complex trauma.
  • Specialist in the treatment of hand conditions, holding a Master’s degree in hand therapy. Extensive experience in thermoplastic static and dynamic splinting, together with grip strengthening, therapeutic exercise and functional rehabilitation.
  • Experience of vocational rehabilitation both as an independent practitioner and within health and social care settings, including work site assessments and development and coordination of return to work programmes.
  • Interventions include managing physical rehabilitation assessments and devising and implementing individually tailored programmes to promote independence in the home and work environment.
  • Experience of treating clients from a variety of diverse cultural and vocational backgrounds, including those with limited English.

Managerial skills/experience

  • Team leader within a large NHS foundation trust including a period of time spent as acting head OT.
  • Responsible for the organisation, management and development of a cost effective OT service for patients with predominantly orthopaedic conditions and diagnoses.
  • Responsible for staff development including supervisory support, appraisal, training, mentoring, performance management and recruitment and selection.
  • Conducted clinical audits, maintained appropriate clinical records and managed resources, supplies and satellite equipment stock.

Independent Occupational Therapist and Case Manager

  • Complete independent assessments for insurance companies for personal accident and personal injury claims.
  • Implementation of vocational rehabilitation to facilitate a client’s return to their previous employment or support them to redeploy to suitable employment.
  • Liaison with treating professionals, consultants, GP’s, physiotherapists and counselling services to coordinate and manage care requirements.
  • Liaison with solicitors dealing with personal injury claims.
  • Work closely with claims managers to ensure that care management packages remain within agreed budgets.
  • Complete accurate, objective case reports within set service level agreements making recommendations for provision of on-going treatment or equipment. Provide regular update reports to inform all parties of case progress.
  • Source equipment and professional services based on individual needs.

Team Leader/Clinical Specialist OT in Hand Therapy and Trauma and Orthopaedics. Burton Hospitals NHS Foundation Trust, Queen’s Hospital, Burton upon Trent.

  • Hand trauma caseload ranging from simple finger fractures to multiple upper limb trauma including fractures, tendon and nerve injuries.
  • Post-operative treatment for elective conditions, for example Dupuytren’s contracture, carpal tunnel release and trapeziectomy.
  • Support and teaching to OT staff with assessments and treatment of clients with complex orthopaedic injuries and multiple traumas. This includes advising and sourcing complex or bespoke pieces of equipment and subsequent liaison with community based colleagues to arrange provision of equipment necessary for discharge.
  • Provision of specialist graded treatment and rehabilitation programmes based on individual needs identified from thorough assessment procedures, using professionally recognised standardised assessments and objective outcome measures.
  • Specialist knowledge of bespoke static and dynamic splints as treatment dictates.
  • Facilitate patients to resume their usual daily living tasks and employment wherever possible and where necessary seek to find alternative or compensatory techniques.
